I've seen reasons such as "insufficient documentation" which puts the responsibility of providing photographs and documentation of the damage on the homeowner.  This can prove to be a challenge if you have a high or a steep roof, or if the damage occurs in inclement weather such as the winter.  It can be extremely dangerous to get on a icy snow covered roof in the winter or a slick roof in high winds in order to to take photos for your insurance company.

Other verbiage I've seen used by insurance companies is "failure to mitigate" meaning the homeowner  didn't take reasonable steps to stop or reduce  a leak.  Most often times this occurs while you are waiting for your insurance to access the problem, determine if they will cover and at what rate they are willing to cover, find a contractor, etc. In the mean time, any damage that occurs inside your home from the leak is not covered.

And my all time favorite denial reason for insurance companies not covering roof damage is because it was "an act of God". Lightning strikes a tree and the branches come down and damage your roof, guess what? That's an act of God and your insurance is not going to cover the damages.
